The conversations are also different. You know, there's this biohacking space of shiny new nickel, new peptides, all this great stuff. But when we have these conversations at the booth, it always comes back to those basics, right?

The basics that we've been talking about for 10 years. Go outside, sunshine, rest, de-stress, eat clean foods, remove toxic burdens. So it all always come back those basic.

And so I love bringing people back the basics. Especially with some of the conversations around things like AI, which has come into our space. This is fairly new for both of us coming into this, which is a very helpful tool and clearly the future.

However, the debates I've been having at the booth and the discussions that we've be having one-to-one with doctors is around AI and how to use it in medicine.

Because let's face it, a lot of projected protocols in conventional medicine to come to understand that this is personalized approach to medicine, everyone's different.

It's not a check mark of a protocol that's created somewhere else to make your life easier. Hello and welcome back to the Evolution of Medicine podcast.

We are here at the Institute for Functional Medicine's annual conference, and we are connecting with leaders and innovators at front lines of healthcare transformation.

And here is Dr. Patti Devis. She's the chief clinical officer at Genova. Uh, we've been friends for 10 years. Yes, followed each other's journey along the way.

Yeah. Maybe we could just start with your journey to be here, at this conference and a little bit more about how you got here. It's always great to see you, James.

As I said, we've evolved with medicine over 10 years. I was originally a conventionally trained internal medicine doctor, and I worked in a hospital for 20 years and left because people weren't getting better.

They were getting sicker quicker. So I left and found Genova. Thank goodness. Learned everything I could about functional medicine voraciously. Became the chief clinical officer.

And I've been there for 10 and have gotten to know you. Come to all these conferences. years, four decades. This is our 40th year. And with that, we've seen a lot.

We've evolved a lots. we have grown and we lead. So we been a big supporter of the Institute for Functional Medicine for many, many years. For us, it's always fun to come to support IFM, to hear the key opinion leaders giving us the latest and the greatest, meet new friends and find our old friends, and so we're honored to always be here.

Yeah. So you've got a good pulse, I think, of how people are feeling. I've noticed there's a ton of new people here, but I'm just wondering what you're, as someone who's like having conversations, what are you hearing this year and what's interesting to you?

I'm so excited. Over the past five years, since COVID, the time of COVID where people are trying to take better control of their health, there's been a real focus and a shift to what we do, which you and I have been trying, to shelter us from the rooftops for many, many years.

So we're seeing a lot of new people come into the space, not leaving conventional medicine, but knowing that there is something more and coming here and really genuinely trying understand.

It's not only the fact that we are getting some newer people into this space which I love, But the conversations are also different. You know, there's this biohacking space of shiny new nickel, new peptides, all this great stuff.

But when we have these conversations at the booth, it always comes back to those basics, right? The basics that we've been talking about for 10 years.

Go outside, sunshine, rest, de-stress, eat clean foods, remove toxic burdens. So it always comes back to those basics. And so I love bringing people back the basics, especially with some of the conversations around things like AI, which has come into our space, right?

This is fairly new for both of us coming into this, Which is a very helpful tool and clearly the future. However, The debates I've been having at the booth and the discussions that we've be having one-to-one with doctors is around AI and how to use it in medicine.

Because let's face it, a lot of us projected protocols in conventional medicine to come to understand that this is a personalized approach to medicine, everyone's different.

It's not a check mark of a protocol that's created somewhere else to make your life easier. And so when you're taking lab results and you putting it AI, and it's telling you what to do, I'm pulling people back and saying, we're treating people.

We're not treating data, we're treating a person. So those conversations, these nuanced conversations as things are evolving with AI and with the newer things coming into the space, I think are really important debates to have.

And I just am loving having them and hearing different people's perspectives. That's kind of new. Before, it was almost like green allopathy when we both first started in this.

It was lab testing and supplements. We kind took the conventional way and said, test, here's a supplement. Now there's all these nuances that are happening that I love to debate.

Yeah. Yeah, that is exciting. Well, you know, I think one of the things that I just heard Dr. Bland, give his keynote, and it's like a lot of ideas that were impossible 30 years ago.

It's, like, hey, personalized medicine is a joke. You can't do it. you have to do for the standard because it'll be way too expensive. Otherwise, well, now I went to a talk on the continuous glucose monitor for people outside of diabetes.

And here's real time feedback that could lead to not only personalization, but also like the transformation of consciousness of the patient to understand how they interact with certain foods, how to interact their stress.

Data is compelling. It's compelling, great. You know, when you started, but it's the principle of what's involved, right? Love that, love that. And, you know we're kind of like, the bleeding edge type of people.

Not, like we are cutting edge, not bleeding-edge in general. We're the people who are thinking outside the box. Here's conventional medicine, we look at things differently.

So as these things come into the space, I love watching it evolve, you know, gain acceptance in the conventional space. So that's been really exciting for us.

And to your point, data is very compelling. Patient is motivated to change once they see this in real time, what's happening to them. Yeah. Well, I mean, one of the things I've been focused on for the last year and a half, two years is really trying to understand what is happening at the cross-section of functional medicine and Alzheimer's dementia care.

And it's been a revelation to me to really understand the degree to which, let's say, in the Avanthea trial that just happened, everyone does the Genova test.

And there's information on that Genoa test and some of the different tests that inform the strategies to reduce the systemic chronic inflammation that exists that is associated with dementia.

It must be interesting for you, given that You know, I think Genova's evolving right from a company that was really about this one test that you started with right through to now having a range of specialty testing and that allows you maybe to have impact in different areas of the physical body.

thousand percent. That infantile trial was profound, especially for this space to prove out what we've all been doing for years now. Genova is just honored to have been invited to even be part of it.

And the fact that this is going to revolutionize our space, to say, like, no, this stuff is real, and our Genovatests have contributed in any way to that is very exciting.

our basics. We have known, using our specialty lab testing, that if you optimize your microbiome, if your optimize nutrition, all these downstream effects will happen.

But again, it comes back to those basics, but to be proven out in something like what True Neuro is doing in the Avanthea trial, to even be involved is just so exciting.

So we're honored. The thing about the microbiomes is that it's impossibly complex in a certain way. And so you need these kind of tools like ATI and that kind.

I'm just trying to make sense of it because like, I think it's amazing that clinicians in our space have been able to wrap their head around something that one is evolving.

Two, I thinks someone said earlier, there's more species or more microbes in the gut than stars in universe. I mean, the fact that you're dealing with something has that much like complexity and understanding and also symbiosis, and things like, I love things, like quorum sensing, where they're not just acting individually, they are working together, that level of complexity is wild.

But even with that, even following the microbiome, Genova, for 40 years we've been looking at stool And to see how that's evolved in the technologies and the assays and what we can measure, it started to become just this laundry list of bacteria.

Now we're understanding it's not just the laundry lists of the bacteria, It's what they're doing, how they are moving together, what their making based on what you're giving them.

And so when you start to realize how you modulate the entire microbiome it becomes so exciting and it is still a moving target. But to have been studying this for 40 years and now to things like this proven out in this Avante trial is very exciting for us.

Yeah. What are you, what has been some of the things that you've learned along the way about how to make these kind of ideas accessible to someone who doesn't really have much context, never ran a stool test?

So thanks to our mission partners. Let's get back to the content. It's a hard buy-in for someone who's never done a stool test, let's be clear. But I think once you start to explain to them that all disease begins, the microbiome is really the key to just about everything that's going on, including Alzheimer's dementia.

I you think you you to start explain that these bugs are really in charge and what they're doing is important based on what you're getting them. So it's this symbiosis to your point.

And so once you start to explain to them the importance of this, of what can be done, we hear success stories literally every single day because we talk to doctors every day.

And we know the important and we can do to make people's lives easier. That's really at the end of the day what we want. We want to help people. So to your point, the microbiome is so important.

As we continue to evolve we're helping more and more people because at end the of day that's the answer. It's a key. I'd love to just go into that a little bit because I see, you know, now I've been involved in this project where all of these things are being measured, a few times.

You've got like biomarkers, which reflects sort of like the biochemistry, right? You got sort CGMs and wearables that have a bit more of a dynamic piece.

But uniquely here in the microbiome, You have sort like a measure of an ecology, Right? It's an ecological understanding. And it seems to me that the ecological understanding might be the key thing, because at the end of the day, functional medicine is an ecological way of practicing medicine.

It is. And more recently, stool testing, like I said, methods have evolved. Genova now does just about every method you can do in stool tests. We have whole genome sequencing, which is super exciting, looking at the entirety of the DNA ecosystem of GI tract.

Not only who's there, but their potential to make certain things. Now you're pairing it with something like the GIFX where we're showing you what's being made.

We're looking at those discrepancies and saying, no, this is a deeper understanding of you have a potential to make something, but your level here is low.

Why? So the level of understanding gets deeper and deeper, and more impactful. So it's very exciting. And I love being on the cutting edge of this stuff.

Yeah. I wanted to share with you a story that's happened to me in the last little while. I came to find out through, actually it was in Mother Jones Magazine, that essentially the land above my house, so I live in El Dorado County, which is in the foothills of the Sierra Nevadas, and there was a terrible fire there actually the year that I moved in called the Caldor Fire five years ago.

So there's basically this 2 million acre burn scar that went all the way up there. Now they're in a process of reforestation, right? And so they wanna bring the forest back.

But rather than just saying, well, Nature's been doing this for billions of years and probably knows how to do it, right? How to reconstruct it. Yeah.

The answer for organizations that are really focused on conifer growth is to use glyphosate to be able to kill all the weeds so it doesn't compete with the conifers so the confers can grow quickly.

And I had a friend of mine who is an organic farmer. He goes to the same school as my kids. Our kids go to school together. And they said, look, can you make a video on this?

Because I've made a videos saying, hey, go protecttahoe.com. Let's fight this because it's insane. Oh, if you put glyphosate up there, what's happening to my stream, my well, water, food, organic food?

So anyway, so he creates this video and he's talking about all the things that you should do to repopulate. And none of those things are glyphosate or killing anything.

It's all about recreating the good things. I just, as he was speaking, I was like, this is like microbiome regeneration. And I was thinking about when we were going to have this conversation because it was just so obvious.

I'm like, oh, yeah, that sounds exactly the same. You could watch that video and say, OK, well, what's it like to be a regenerative farmer? Well, it's the exact same thing as being a functionalist.

That's exactly it. It's letting the soil do its thing. That's shocking. It's a very strong parallel to what happens in the microbiome. Once you're on antibiotics, for example, to rebuild that microbiom in a healthy and safe way rather than not.

But that's And one of the things he said is that he's said, look, you know, if you kill all everything, then the thing that grow back are the resistant things.

And I'm like, that's like Mercer up the hill from me. Don't bring your Merce down here. You know what I mean? So crazy. There's that ecological way of thinking.

I think part of what happens when practitioners come to functional medicine, they learn about Genova, and they go through this and start to think in ecological terms.

Once you've had that thinking, you can't shake it and you realize that not thinking in that way is like the root of all of our problems. And that's kind of where I say like we always go back to the basics, like all new supplements or peptides are coming out.

It's like it's the basic. There's those basic concepts that we continue to hone in on because to your point, The ecosystem of your microbiome is like soil and you have to feed it properly and things out of the way to prevent it from growing like glyphosate.

So I love that analogy and that's something that we hone in on all the time. It's the ecosystem, the microbiomes being healthy. Dr. Michelle Pero, who's a pediatrician in California, spoke at one of my events recently.

She was talking about particular strains of microbes just not being present at all in certain children based on historical glyphosate exposure. It's very interesting to just see the long-term biological impact of those ecological interventions, and also that there's sort of like the transgenerational effect too, which I think Jeff was talking about in his lecture.

Yes, he is. And it's that developing microbiome that's so important, little kids reintroducing foods out in the dirt, who you're around, here you are with.

I just, I hope everything's going to be okay by your house. And a fight. Good. Yeah. So one of the things that is happening, I'm really excited about this collaboration and I want to thank Genova.

Thank you for your support of this and doing this. I hope we get to do it again. It's been amazing to meet so many people and to create like essentially 10x more content than I used to.

Create and make one show from the show and now we've got it's an opportunity to see so One of the things that, there's two sort of forces happening that I think I wanted to ask you about.

One is this sort growth in direct-to-consumer lab testing, where people are realizing that they have a very small amount of data on themselves. Interesting fact, I was speaking to a doctor here who's from Japan, and they were saying that their annual panel that the get every year is much more significant here.

That's just like the normal nationalized medicine, they're getting a lot more of that. But here, so now hundreds of thousands of people are choosing to get a little bit more data about themselves, which is really interesting.

And full script journeys is creating a way to essentially for practitioners to do that. I hear that at some point very soon, Genova will be able to be added into those full-script journeys so that, at the top of the funnel of a patient interaction, a person could choose to have an ecological understanding of their gut.

That's right. the implications of that, the benefits of and what might be, you know, what may be the implication of be for clinics and for communities.

Well, I think it comes back to what I was saying earlier that I'm just deeply heartened that there are people out there that are trying to take better control of their health, right?

They're being more proactive. They are going to direct to consumer platforms, which, you know, Genova's got one, genovaconnect.jdx.net. Fullscripts is great, great.

We're talking with them about journeys, like specific clinical conditions. What does that journey look like? What testing might be helpful to you? And I thinks patients resonate with this, And also, they may not have the depth of understanding and the ability to read the results the way that they should and to actually do something with it.

That's a big barrier, one that we're trying very hard to overcome with education. And our platform is called Junova Connect for a Reason. We're connecting patients with clients on the back end that can actually help them in their journey.

They may have started with a direct-to-consumer, but we want to connect them with an expert in and also putting out more content to help educate some of these patients who are seeking help to have it more accessible.

trying to put some of our educational content on there, again, to help both clinicians and patients because we do understand that patients are searching and they're frustrated and we want to them and connect all of these people together in community to your bigger point.

And I will say, Juneau's got a podcast. It's called The Lab Report, co-hosted by me. Whenever James is on, we just are jumping out of, waving our arms.

This is how I know James. He's so inspiring. I think it's helped a lot of people to really understand how Can I tell a funny story about that? I came on the podcast the first time and I did it.

And I showed up at a conference like this and someone comes up to me and goes, cheese and mayonnaise sandwiches. I can't even believe you eat that. It was the banh mi.

That's what it was. The banhi mi is the best sandwich. But you know, I do burp on cheese. So great. You have to go listen to these episodes if you've missed them.

James was on fire. We were jumping up waving our arms. It was just so inspired by everything that you did. And you just really brought me along and inspired me on the way.

So I appreciate you as a friend. Well, one of the things that's happening that I see, and this is sort of what the work that involved in, is the vision that see for the next phase of functional medicine revolution is what I'm sort thinking about is like organized clinical networks.

And I would say the one that focused on starting with is cognitive decline, because I think there's an opportunity to sort normalize the standard of care that can be agreed upon, which isn't what has happened in functional medicines, rather than keep a standard care, there's been this sort of emergent phenomenon of actual medicine, which creates the space for innovation to happen and happens.

But ultimately, what that means is that everyone's doing it a bit differently, right? Which doesn't really allow the sort necessary cohesion to be a, you know, a specific service.

And so I'm focused on that. an obvious clinical network, but I could imagine coming down the road would be a gut health, no clinical networks. And I did a podcast recently where I found out, I should have heard this already, there's like between 25 and 45 million people with IBD.

Yes. Yeah. Also, if you listen to that podcast with Dr. Isabella Wentz, you just see that actually, I could see that there's no real way of solving this problem for those 25 to 45 million people without an ecological framework because it's an ecology that you're implementing and that's debating.

And so then it got me thinking like, okay, well, if I had a functional medicine clinic today and I was just starting out, gut would probably be the easiest way because I think the GI medicine thing is mainly out to lunch, you know, that's my opinion, to give that opinion.

But I wouldn't be able to see a GI doctor who didn't understand the body as an ecology. That would not be my choice because it is obviously an oncology, right?

And so then there's this huge gap in the market and lots of practitioners are taking up on that gap. Everyone in functional medicine does do that, but I also do think that It's a niche that's easy to be part of.

And if we said tomorrow, hey, we're starting this gut health nationwide clinical network, almost every functional medicine doctor could be a part said network.

Yes, easy. Ultimately, Genova could a foundational element for that. That data multiplied by a bigger understanding could open up the next phase of understanding what really drives everything.

I agree. Thankfully, I think the paradigm is shifting a tad with the conventional gastroenterologist. I just spoke to a patient a few days ago who has been failed by gastraenterology.

She had IBS, she had traveled, had diarrhea, got scoped from above and below. There's nothing there. Here's some Imodium, you have Ibs. And so for years, this has an ongoing struggle.

And just by educating her and saying, you don't have to live this way. There's more. That there are tools out there. And so I think there's real hope.

To pick up that mantle is going to be some of those functional medicine clinicians who are going fill in the gaps with gastroenterologists and hopefully pull the gastraenterologist along with us in educating them.

So I, I that network will be filled by functional medicines clinicians. What are some of the specialties and specialists that are showing up to Genova today that give you hope that something's happening?

That's a great question. We literally just had a discussion at practice about OB-GYN. The obstetricians and gynecologists in women's health has become a huge thing showing In functional medicine, I mean, just by Genova statistics, and I think across the board in functional medicines, the number one patient that seeks out functional medical are middle-aged women from 45 to 55 who are perimenopausal, you know, hormones are shifting, they go to the primary care doctor who's not really fully understanding, there's a myriad of symptoms a mile long.

They're told they're a little bit crazy. No one can really help them. So they are coming to functional medicine to seek help. I think the Ob-Gyn community is really opening up to this and is stepping up in a big way in addition to the fact that the black box warning has been removed from bioidentical hormone replacement therapy.

They are showing up at the booth, just hungry for more information, so I'm really heartened by that. Like I said, gastroenterologists, we're pulling them along.

But I was hearten by this specific conference of the amount of Ob Gyn doctors that are here. That's cool. What about psychiatry? A huge growing market, huge going market.

Even as I go to conferences and hear lectures, the mental health has become paramount. Ketogenic diets have been mentioned. The gut-brain axis has been mention.

I think that's a burgeoning part of what we're doing here in functional medicine with the rise of all kinds of anxiety, depression, all of these mood disorders.

This is a place that you're going to see big movement in the coming days. Yeah. Yeah, I'm interested. You know, one of the things that I'd love to understand is, you know you guys do all these tests.

And you do repeat tests for people in certain cases. But how do you ever find out what's happening between those repeat test? Like what is the person actually doing?

It's a great question. We get a lot because we speak to clinicians, mainly. So it's, Patty, here's what's going on with this patient, blah, we review the results.

I'm thinking, maybe think about this. Think about that. They'll repeat a test, call back three months later and say, Patti, you were right. Here's What happened.

And so we get these success stories over and over, and that we're so bought in to the power of what we do that it keeps me coming and showing up every day super excited.

It's hard to know specifically what's happening with the patient, what they're doing day to day, other than if there are health coaches that are involved or there's CGMs or something that's collecting data.

That's a different story, but we're kind of limited to what clinicians tell us and what the've intervened with and we are seeing a repeat testing. So that is very exciting for us on the front lines as we speak to clinicians and I'm sure it's even more exciting to the clinicians who are saying the results in front of themselves with their patients.

At the end of the day we want patients better. Well, I wanna follow up on that conversation because I believe that something that I'm seeing happen is, if you look at Genova, or if look a Function Health, all these kind of things, you have repeat testing, but what you don't have is an understanding of what I would call the participatory layer.

And that's the point. Yeah. Between office visits, what's happening? What's going on? And what are they doing? It's also difficult to know. How do you recap what I've done for the last 90 days in a five-minute doctor visit?

Correct. Right? That's basically impossible. The next step is like you have a health coach and the health coaches with them a little bit more time and understanding that.

Then the next thing would be like, okay, a continuous glucose monitor to be able to see for periods of time how their bloodstream was affected. The Next thing might be an aura ring to how the other things were happening.

the Next they might taking photos of their food and then tracking how they were eating and what supplements they would take and do that In the early cases of what's happening with traneura and the reversing of cognitive decline, we're all the way over here.

I've seen it. Some of that has come all over. And some doctors, like I just interviewed Dr. Ryan Arnold, who's here, speaking on behalf of Dexcom, that he has people doing preventative Alzheimer's programs and not even like, they don't even have it yet, but they've done the testing to see where their risk categories are, that they're getting CGM and coaching and inside the software saying, you know, did you poop today?

Did you see the morning light? did You eat these vegetables? And to me, what I see is that like you have so much data inside, But without that understanding the participatory layer, it's hard to know what caused what cause what.

Unlocking that. I think is the game changer. And I've seen your True Neuro program and it's fascinating because there's a lot of data. You're going to take all this data from an oral ring, from a CGM.

It's lot a data to put together. But what TrueNeuro has done, and a lots of programs do, is put it together in a cohesive state so when a clinician meets back up with that patient, they can put as much together as you can.

you Can't capture all of it. Yeah. but you get pretty close. Some people can't get that close, it still an open gap, I Well, I'm just glad Genova is in the mix because it does give us the key insight on ecology and the ecological changes that happen.

And ultimately, you know, these are diseases of broken ecology, one way or another. So I think we'll see that more and more. Well, look, again, I want to say thank you for supporting this to happen.
