Why did it take three appointments before anyone mentioned menopause? Why do symptoms get dismissed as just anxiety? Most primary care providers are not trained to think about it, and OBGYNs are often too busy to fully address it. Menopause is so much more than hot flashes. The data on symptoms shows brain fog, anxiety, and joint pain among the most common, which is exactly why it takes a specialist who is actually looking for the pattern. I am a board certified family physician with a menopause practitioner certification from the Menopause Society, and thousands of hours spent with menopausal women in my training and career. I have also personally dealt with hormonal issues, and watched my own mother go undertreated for years before anyone prescribed her anything, and my grandmother never got treated at all and now lives with severe osteoporosis. That history is part of why I care so much about getting this right. Hormone therapy is not an excuse for doctors to avoid a real conversation. Why a normal TSH and a normal free T4 do not settle the question in a chronically ill patient, and which markers I think tell you more about what is reaching the tissue. How peptides differ from hormones mechanically. Hormones work through the nucleus and change protein synthesis, slow on and slow off. Peptides act largely at the cell surface through secondary messengers, quicker on and quicker off, with effects in more than one direction at once.
