Neuroinflammation is activation of the brain’s immune system—primarily microglia—that releases cytokines (e.g., IL‑1β, TNF‑α, IL‑6) and reactive oxygen species; short‑term this is protective, but chronic activation disrupts neuronal signaling and damages tissue. Which symptoms appear depends on the brain region involved (e.g., basal ganglia inflammation → sudden OCD/tics as in PANS/PANDAS; amygdala → severe anxiety and panic; motor cortex or substantia nigra → tremor, weakness, Parkinsonian signs). Chronic neuroinflammation can also promote accumulation of beta‑amyloid and phosphorylated tau, contributing to synapse loss, cognitive decline, and dementia. Common upstream drivers include infection, toxins, immune dysregulation, and systemic inflammatory states (diabetes, obesity, cardiovascular disease). Emerging imaging (PET/MRI) can detect microglial activation, and clinically it’s important to treat underlying inflammatory drivers because therapies that only target neurotransmitters often give limited relief when neuroinflammation is the root cause.
