That is inflammation of the kidneys' filtering units. But here's what's fascinating. While most kidney doctors treat this as simply a kidney problem, a functional medicine reveals it's actually a story about immune system dysfunction that starts far from your kidneys. So let's dive into what IgA Nephropathy really is, why it develops and most importantly, how we can address its root cause to help your kidneys heal. First, let me explain what's actually happening in IgA Nephropathy. IgA stands for Immunoglobulin A.
It is one of your body's main antibodies. Normally, IgA protects you at mucosal surfaces like your gut, respiratory tract and urinary tract.

It is your first line of defense against pathogens trying to enter your body through these barriers. But in IgA nephropathy, something goes wrong. Your immune system starts producing abnormal IgA antibodies that form immune complexes. Basically, these antibodies stick together in clumps. These clumps then travel through your bloodstream and get deposited in your kidneys, especially in the glomeruli, which are tiny filtering units of your kidneys. Once these immune complexes lodge in your kidneys, they trigger inflammation.
This inflammation damages the delicate filtering structures, leading to blood and protein in the urine, and over time can cause kidney function to decline. Now here's where it gets really interesting from a functional or a holistic perspective. The question isn't just what's happening, it's why is this happening in the first place? The immune system becomes hypervigilant, producing excessive amounts of IgA in response to what it perceives as constant threats.

Traditional autoimmune diseases like lupus or rheumatoid arthritis involve your immune system directly attacking your own healthy tissues. But IgA nephropathy is a bit different. What happens is your body produces abnormally structured IgA-1 antibodies. Specifically, they are missing certain sugar molecules called galectose.
These galectose-deficient IgA-1 molecules aren't foreign substances, but your immune system doesn't recognize them as normal either. So your immune system creates additional antibodies, either IgG or IgA antibodies, that target these abnormal IgA1 molecules. When these antibodies bind together, they form immune complexes that circulate in your bloodstream and eventually deposit in your kidneys, causing inflammation and damage. This is why we call it a immune complex-mediated glomerulonephritis. It has autoimmune-like features, but it's not a full-blown systemic autoimmune disease.
It's more like immune system is confused rather than directly attacking healthy tissue.

As for genetics, there definitely is a hereditary component, though it's complex. We see IgE and Afropathy running in families, especially in East Asian populations, where it's most common. But it's not a simple genetic inheritance pattern like some diseases. Instead, it's what we call polygenic and multifactorial. This means multiple genes are involved and environmental factors play a crucial role in determining whether someone with genetic susceptibility actually develops the disease. Some families might show patterns that look like autosomal dominant inheritance with variable expression, but this isn't universal.
Scientists have identified several genetic variations associated with increased risk, including genes involving the HLA system, complement pathways, and most importantly, genes that control how IgE is processed and glycosylated that sugar attachment process I mentioned earlier. But here's the key insight from functional medicine, even with genetic predisposition. Genes load the gun, but the environment pulls the trigger. Having genetic susceptibility doesn't mean you're destined to develop the condition.
It means you're more vulnerable to certain triggers. But when we identify and address those triggers, we can often prevent the condition from progressing or even help it improve. The most common triggers I see in my practice include chronic infections, particularly upper respiratory infections, which can stimulate excessive IgA production. Stress is another major factor. Chronic stress dysregulates the immune system and can worsen intestinal permeability. Environmental toxins also play a role. Heavy metals, mold toxins, chemical exposure can overwhelm your body's detoxification systems and contribute to immune dysfunction.
