What Really Happens When You Tear Your ACL?
My friend asked me recently... "Are ACLs just made to tear?" It's a wonderful question, given that we only hear about our favorite athletes and ourselves sustaining this type of injury. But an ACL is meant for so much more, which is why we often reconstruct the ligament. An ACL tear is a rupture of the anterior cruciate ligament, which is one of the key stabilizers that control forward movement and rotation of the shin (i.e.: tibia) on the thigh bone (i.e.: femur). It often happens with a sudden pivot, plant, or deceleration, which is common in cutting sports like football and soccer. In Jiu Jitsu, common mechanisms of ACL injury are from standup, being held in a lockdown, and leg entanglements. Diagnosis relies on a thorough clinical evaluation, with hallmark signs including acute swelling, instability, and the sensation of a “pop.” MRI is used to confirm the extent of the injury.

As a doctor, I get this question all the time: “What exactly does the ACL control in the knee?” The answer: The ACL stabilizes anterior translation and rotation of the tibia on the femur. In simple terms, it keeps the shin from sliding forward and helps the knee handle pivoting and cutting movements, which is critical in sports like football, soccer, wrestling, and jiu jitsu. When this ligament is torn, athletes often report a “pop,” sudden swelling, and instability. In jiu jitsu specifically, I see it happen most with takedowns, lockdowns, and deep leg entanglements. How do you know if you tore your ACL? In traditional sports, ACL tears often follow the textbook pattern: A sudden pivot, a loud pop, immediate swelling, and a feeling of instability. But for jiu jitsu athletes, it’s often more subtle. There’s no big pivot shift or pop, just a heel hook, a takedown, or a scramble that leaves your knee feeling... off. As both a sports surgeon and jiu jitsu athlete, who has also sustained an ACL tear of my own, I see them misdiagnosed or, more frequently, ignored. When we delay diagnosis, we delay recovery. A good example is Nicky Ryan (B Team Athlete). He went 3 years on a torn ACL and decided to eventually get it treated, likely due to continued instability, swelling, and difficulty with the knee.
