What is the Wiley Protocol, and how is it different from conventional hormone replacement therapy? In this video, we explore the Wiley Protocol, bioidentical hormone replacement therapy (BHRT), cycling hormones, and the roles of estradiol and progesterone during perimenopause and menopause.

The Wiley Protocol is a form of compounded bioidentical hormone therapy that uses topical estradiol and progesterone in doses that rise and fall over a 28-day schedule. The goal of this rhythmic or “biomimetic” approach is to approximate some of the hormonal fluctuations seen during the reproductive years. This differs from many commonly used menopausal hormone therapy regimens that provide a relatively consistent estrogen dose, although conventional hormone therapy can also use sequential or cyclical progestogen schedules.

We’ll discuss why estradiol and progesterone are important, what practitioners mean by physiologic hormone cycling, and why some clinicians and patients are interested in hormone regimens that change throughout the month rather than remaining at the same dose every day.

We’ll also look at an important question: Does mimicking a younger woman’s hormone cycle provide an advantage over standard HRT? Although rhythmic hormone dosing is an interesting clinical concept, high-quality evidence showing that the Wiley Protocol is safer or more effective than established menopausal hormone therapy is lacking. The Wiley Protocol uses compounded preparations, and major medical organizations recommend FDA-approved menopausal hormone therapies when appropriate because their safety, potency, and effectiveness have been evaluated more systematically.

This discussion may be especially helpful if you’re researching:

• Wiley Protocol hormones
• BHRT and bioidentical hormones
• Perimenopause or menopause hormone therapy
• Cycling vs. continuous hormone therapy
• Estradiol and progesterone
• Hormone optimization and individualized HRT

Hormone therapy should always be individualized. Age, symptoms, whether you have a uterus, medical history, cardiovascular and clotting risks, breast and endometrial health, medications, and other factors can affect which options may be appropriate.
